Anthem won't cover it? - pediatric cover letter
Anthem is not used for my child, CBC and blood cholesterol levels by the CPT code of the test. Has anyone else this problem? Does anyone know the CPT code and for children (under 6 years), or Pediatric Radio-Canada and cholesterol?
